How do I register for a vaccine appointment?
- Visit the eligibility tool at: am-i-eligible.covid19vaccine.health.ny.gov
- Note: Do NOT use Internet Explorer
- Fill in your eligibility information
- If you are eligible, you will be able to set up an appointment at a nearby location
- SAVE YOUR APPOINTMENT INFORMATION
- Fill out the vaccination form before your appointment: forms.ny.gov/s3/vaccine
What considerations should I make?
- Talk to your doctor if you have…
- Contracted COVID in the past 90 days
- Received other vaccines within the past 2 weeks
- Any autoimmune illnesses, or medications that suppress your immune system
- A history of allergic reactions to vaccines
- Bring ID, your appointment confirmation, and a copy of the COVID-19 form (step 5 above)
- There will be a 15-minute monitoring period after your vaccination; 30-minutes if you have a history of vaccine reactions

If you are eligible for the vaccine:
If you are age 65 to 75 or are eligible under another category in Phase 1a or Phase 1b, you can sign up for the Health Department’s Alert system, which will alert you when more vaccine doses become available.

COVID Rent Relief Extension Program
The COVID Rent Relief Extension Program will provide eligible households with a one-time rental subsidy that will be sent directly to the household’s landlord. Applicants will not need to repay this assistance. The Covid Rent Relief Extension Program is not first come, first served. Applications will be accepted throughout the application period. HCR will prioritize eligible households with “greatest economic and social need.”
For the next round of applications, tenants will have to meet all of the following criteria:
- Must be a renter and have a primary residence in New York State;
- Applicants must have lost income during the period of April 1, 2020 to July 31, 2020;
- Before March 7, 2020 and at the time the application is submitted, household income must be at or below 80% of the Area Median Income, adjusted for household size; and
- Households must be “rent burdened” during the months they are applying for assistance between April 1, 2020 to July 31, 2020.

There are NO home testing kits
If you are called with an offer for a kit – hang up.
It is a scam. There are also a number of phishing emails (i.e., attempts to steal your password) purporting to offer information on COVID-19 from authoritative sources. Don’t click on the embedded links or attachments.

- Wash hands often with soap and water for at least 20 seconds. If soap and water are not available, use an alcohol-based hand sanitizer with at least 60% alcohol.
- Avoiding touching your face, nose, or mouth with unwashed hands
- Avoiding close contact with people who are sick
- Cleaning and disinfecting surfaces that you frequently touch
- Covering coughs and sneezes with a tissue. Then throw away the tissue and wash your hands.
